Lagos State Mandates Health Insurance: What Residents Need to Know

Olamide

Lagos State has announced a December 31, 2026, deadline for mandatory health insurance enrollment, with enforcement set to begin in early 2027.

The Lagos State Government has officially mandated that all residents secure health insurance coverage by December 31, 2026. Governor Babajide Sanwo-Olu announced the policy, which stems from Executive Order EO/BOS/01 of 2024, during an event in Victoria Island. According to the government, enforcement of this compulsory coverage will commence on January 1, 2027.

Understanding the New Policy

The initiative is designed to ensure that all residents can access medical services without facing catastrophic financial hardship. Governor Sanwo-Olu emphasized that the policy requires participation from individuals, employers, and workers alike. The government has categorized these efforts into two primary streams:

  • Ilera Eko Scheme: This existing program remains the primary vehicle for providing coverage to vulnerable populations and those working within the informal sector.
  • Lagos Private Health Plans: A newly introduced framework specifically designed to provide insurance coverage for salaried employees and corporate organizations.

The governor urged employers to view healthcare contributions as a fundamental business obligation, similar to pension payments.

Addressing Healthcare Access and Emergency Care

A critical component of the governor’s directive concerns emergency medical services. Sanwo-Olu stated that healthcare providers are strictly prohibited from denying urgent medical attention based on a patient’s inability to pay or lack of insurance. Under this mandate, providers must prioritize the stabilization of patients before addressing financial or insurance-related documentation.

To facilitate these changes, the state has partnered with 21 health maintenance organizations to expand the reach of the new private plans. Furthermore, the Lagos State Health Management Agency (LASHMA) has increased its Health Equity Fund from N1 billion to N3 billion. This fund is intended to provide coverage for over 500,000 vulnerable residents who cannot afford standard premiums.

The Rationale Behind the Mandate

Lagos State Commissioner for Health, Prof. Akin Abayomi, noted that the rising costs of healthcare—driven by advancements in medical technology, robotics, and artificial intelligence—necessitate a shift away from out-of-pocket payments. The current reliance on direct payments places significant strain on both households and medical facilities.

LASHMA Chairman Dr. Adebayo Adedewe confirmed that the agency is scaling up its outreach efforts. This includes public awareness campaigns across radio, digital platforms, and billboards, as well as direct engagement with traditional leaders, market associations, and transport unions to ensure widespread compliance.

Conclusion

While the state government has set a clear timeline for universal health insurance coverage, specific details regarding the penalties for non-compliance after the January 1, 2027, enforcement date were not provided in the initial announcement. Residents and employers are encouraged to engage with the LASHMA-approved plans to ensure they meet the upcoming deadline.
